Our SNP plan 014 offers extra care for dual eligible members.

Special Needs (SNP)

Our Special Needs Plan (SNP) provides coverage for individuals who need extra help accessing care. The plan is designed to meet the need of people who receive certain Medicaid benefits.

The SNP plan provides additional benefits like transportation services, hearing aid supplements, an over-the-counter allowance for health products of up to $1,000 every year and up to $2,500 in dental coverage.


Integrated Care Management Program

CHPW Medicare Advantage provides Integrated Care Management Program services to all MA Special Needs Plan 014 (HMO SNP) members.

An IMC specialist will assist you and your health care team in transitions of care and maximizing the resources available to you.

You will receive a letter with a health risk assessment (HRA) when you enroll and again annually to help us identify areas that we can assist you in optimizing your health care needs and support.

We are committed to helping SNP members transition from one care setting to the next, and back to their medical homes. Your care management specialist will answer any questions you have about changing care facilities.

Behavioral Health Services Only

This program is for dual eligible Medicaid and Medicare members (or those who pay service fees). This plan provides coverage for mental health services only.

Health plans administering Integrated Managed Care also offer Behavioral Health Services only plans. If you are eligible for Apple Health coverage on a fee-for-service basis, you may choose a Behavioral Health Services Only plan. For clients who do not select a plan, HCA will automatically enroll them into a BHSO plan. A BHSO fact sheet is available online.
