Transportation Benefits and Support

Rides for Dual Complete (HMO D-SNP) Members

Our Dual Complete (HMO D-SNP) offers a transportation benefit of up to 40 one-way rides (up to 50 mile limit) per calendar year.

Need to get to a doctor’s appointment or see your pharmacist? Use these rides to get to and from provider offices, medical centers, and pharmacies.

Schedule your next trip by calling us at 1-800-942-0247 (TTY: 711), 8:00 a.m. to 8:00 p.m., seven days a week. We can accommodate wheelchairs and other accessibility needs.

Everyday Needs for All Members

For transportation for your daily life activities, like getting groceries, we can connect you with local organizations that assist seniors with these needs. Transportation vouchers for public transit or gas may be available.

To request assistance, contact your Case Manager, or call Customer Service at 1-800-942-0247 (TTY: 711), 8:00 a.m. to 8:00 p.m., seven days a week.

Emergency and Ambulance Services by Medicare Advantage (MA) Plan

If you are having an emergency, call 911.

Note: Costs below are for ambulance and/or emergency services. Please see above for other transportation benefits, such as rides to medical appointments or assistance getting to the grocery store.

Ambulance services for emergency, non-emergency, and AIR ambulance are covered per one-way Medicare-covered trip.
