- Community Health Plan of Washington was founded in 1992 by Washington’s Community Health Centers. We became the first not-for-profit managed care plan founded by Community Health Centers in the state.
- Our mission is to provide services that support the health and well-being of our members, both directly and through partnerships with our valued community-based providers.
- We work closely with local providers to ensure your needs are being met, because your whole health is much more than just a visit to the clinic or doctor’s office. We step outside the clinical setting to help connect you with resources in your community.
*The plan premium of $36 a month is paid for by Medicaid or because you qualify for Extra Help. Your Medicare Part B premium must continue to be paid, although that too may be paid for you through subsidies.
