Print ID Card

Always carry your CHPW Medicare Advantage ID card with you. You will need it when you see a doctor or other health care provider or fill a prescription.

Need an ID card?

If you need a replacement card or want to print a temporary version, login or create a new account in the member center. If you are having trouble logging in, click the “Forgot Password” link to reset your password. You must know your username in order to reset your password. If you forgot your username, you may need to make a new account. 

PRINT ID CARD

If you are new to CHPW Medicare Advantage, it takes about a week to receive your ID card from the time you signed up for your plan. Your ID card might be on its way.

If you do not have your ID card, our in-network providers, hospitals, and pharmacies can confirm your coverage directly with us, so don’t hesitate to get care if you need it.

How can I reset my password if I forget my username?

You need to call customer service at 1-800-942-0247 (TTY Relay: Dial 7-1-1).

I just enrolled, when will I get my ID card?

You will receive your ID within 30 business days. If you need your card now, you can print a copy from your myCHPW account.

I just got a new ID card, but my new and old cards say the exact same thing. Which one is valid?

Although the information may be the same, your new card is the one that is valid. You may discard the old card and you will not be able to use when seeing providers as conditions may have changed.

Is there a separate ID card for dental or vision?

Your CHPW ID card is the only card you need and can be used for medical, dental and vision.

My information is incorrect. How do I fix it?

If the information is incorrect on your current ID card, please call Customer Service at 1-800-942-0247 (TTY Relay: Dial 7-1-1) from 8:00 a.m. to 8:00 p.m., seven days a week.  You will need to print a new copy of your ID card once your information has been updated or you can ask Customer Service to send you a copy by mail.

What information is on my ID card?

Your ID card contains your member number, name, and information about your primary care provider.
