Community Health Plan of Washington is an HMO plan with a Medicare contract. Enrollment in Community Health Plan of Washington Medicare Advantage Plan depends on contract renewal. To enroll in a Community Health Plan of Washington Medicare Advantage plan, you must be eligible to receive Medicare Part A and Part B and live in our service area (Adams, Benton, Chelan, Clallam, Clark, Cowlitz, Douglas, Franklin, Grant, Grays Harbor, Jefferson, King, Kitsap, Lewis, Mason, Okanogan, Pacific, Pierce, Skagit, Snohomish, Spokane, Stevens, Thurston, Wahkiakum, Walla Walla, Whatcom, and Yakima counties in Washington State). Based on a Model of Care review, Community Health Plan of Washington has been approved by the National Committee for Quality Assurance (NCQA) to operate a Special Needs Plan (SNP) through 2022.
