CHPW has rolled out a newly improved Care Management program to better support the needs of our Medicare members, your patients. Particularly those most at risk due to complex conditions or unmet health or social needs. As a valued provider in our network, you play an important role in the success of the program. We hope we can count on your active participation!
With our new Care Management approach, we aim to establish strong connections with these members/patients early on to ensure that their care is coordinated, and health is optimized from the start. We will assign each member/patient a dedicated CHPW care team member who will work directly with them and their primary care provider and clinic.
About our team
The Care Advocate is a new role and is the face of the care team for the member and works to support effective communication between the member and health care providers.
Although Care Management programs are available to all CHPW members, our newly designed program and services focus on addressing the needs of our Medicare Advantage and D-SNP (Dual Eligible Special Needs Plan) Medicare Advantage members.
More about our new Care Management Team
The CHPW care team will include the four C’s:
- Care Advocate who is the member’s main care team contact at CHPW
- Case Manager to help coordinate care for complex members.
- Clinical Care Coordinator to help coordinate care for members with less needs and lower complexity.
- Community Health Worker to connect the member to local resources for everyday needs like food and transportation.
Each of our network of 21 Community Health Centers will have a CHPW care team assigned to it to optimize collaboration and coordination for our Medicare and D-SNP members and their providers.
We need your help
As a provider, you can help by contacting your Care Team to refer CHPW-insured patients to our Care Management program. Encourage your staff to work with our CHPW Care Team. Your staff can also support the patient by participating in interdisciplinary care team communications to assess how the member/patient is doing and provide recommendations and feedback.
For everyone involved, the roles may vary but the end goal is the same: to help improve the health of the member/patient.
