Can I use my Member Service Center username and password for myCHPW?
We’ve moved all the tools in the Member Service Center to myCHPW. Now you can see your ID card, claims, referrals, prior-authorizations, plan documents, and more – all in one place with one sign-in.
If you haven’t signed into myCHPW in the past, sign up and create a new username and password to enjoy your updated member portal. You will need your CHPW member ID number. Sign up ➔

I forgot my password
If you know your username and the email you gave when you registered for myCHPW, you can easily reset your password.
- Go to mychpw.chpw.org, select Forgot Password?
- Enter your username
- Answer your three security questions to verify your identity
- Check your email for a one-time password (OTP). Copy or write it down.
- Select the link in the email to change your password.
- Enter the one-time password and a new password.

I’m locked out of my account
To help keep your health information secure, if you enter your password incorrectly three times, your account will be locked.
To unlock your account, call our Customer Service Team at 1-800-942-0247 (TTY: 711) seven days a week, from 8 a.m. to 8 p.m. If you know your CHPW member ID number, complete a short form to send us a secure message.

Where do I get my member ID?
The member ID you need to register is on the front of your CHPW member ID card.
If you don’t have your CHPW member ID card, our Customer Service Team can help. Call 1-800-942-0247 (TTY: 711) seven days a week, from 8 a.m. to 8 p.m.
Why isn’t my member ID working?
There could be a few reasons why your ID isn’t working during sign up:
- Did you recently change your CHPW health plan?If you had an Individual & Family Cascade Select plan or Apple Health (Medicaid) and recently became eligible for and enrolled in a CHPW Medicare Advantage (HMO D-SNP) plan, your CHPW member ID will change. You will get a new CHPW member ID card in the mail.
- Is your health plan or coverage active? To register, your coverage must be active, and you must still be eligible for your plan. Not sure? To check if your coverage is active, call our Customer Service Team at 1-800-942-0247 (TTY: 711) seven days a week, from 8 a.m. to 8 p.m.
View or change my doctor
Who is my doctor (primary care provider)?
You can see who your primary care provider is in your myCHPW portal.
- Sign in
- Select My Care at the top of the screen.
- Under My Care select Change PCP/Clinic.
- Under Current Doctor you’ll see your doctor’s name and your clinic.
How do I change my doctor or clinic (primary care provider)?
There are two ways:
Main menu (navigation)
- Sign in
- Select My Care at the top of the screen.
- Under My Care select Change PCP/Clinic.
- Search for doctors in your current clinic or by doctor name.
- Follow the prompts to select your doctor.
Dashboard (home)
- Sign in
- Look for a box that says Your Clinic.
- Select Change clinic.
- Next you’ll see Choose Your Primary Care Provider.
- Search by clinic or doctor to select a new doctor.
How do I search for in-network providers?
- Sign in
- Select My Care at the top of the screen.
- Under My Care select Find a Doctor.
- Enter your Zip, City, or County.
- Select Search.
ID card, costs, and coverage
How do I save or print my ID card?
You can save or print your CHPW member ID card in your myCHPW account.
- Sign in
- On the Dashboard (home) select CHPW Member ID.
- To download, save, or print select Open.
- Your ID card will open in a new web page or in Adobe Acrobat Reader.
- Look for the icons (small pictures) at the top and select what you want to do:
Where can I see what’s covered?
To see which health services are covered by your health plan, Search Documents & Letters for your Summary of Benefits and Evidence of Coverage.
- Sign in
- Select Self Service at the top of the screen.
- Select Search Documents & Letters.
- From the Document Center under Document Type select Plan Documents.
- Select Search.
- Under Search results, look for Summary of Benefits for an overview and Evidence of Coverage for details about your plan’s rules.
- Select View or Download.
If you’ve requested audio format, you will also have a separate audio file (.WAV) you can select Play to listen to the file.
Where can I check if a health care service I received was paid for (claims)?
There are two ways:
Main menu (navigation)
- Sign in
- Select Claims & Pre-approvals at the top of the screen.
- Under Claims & Pre-approvals select Claims History.
- Under Claims select a time frame or All to see the most results.
- Select Search.
- To see details for a specific claim, under Search results select the Claim Number.
Dashboard (home)
- Sign in
- Scroll down to Recent Claims.
- Preview claims from the Dashboard or select All Claims to view a full list.
- To see details for a specific claim, under Search results select the Claim Number.

Where do I update my contact information?
You can update your contact information or change your communication preferences in your myCHPW account.
- Sign in
- Select Self Service at the top of the screen
- Under Self Service select Communication Preferences
- Under Contact & Communication Preferences select Edit
- Enter your information and choose your preferences then select Save

What is two-factor authentication (2FA)?
Two-factor authentication gives your account another level of security. If you set it up, each time you sign in, you will be required to enter a one-time passcode that is sent to your email, in addition to entering your username and password.
Two-factor authentication is not required. When you sign in, you may be asked to set it up. If you don’t, you can still access and use myCHPW.
To set up or turn off 2FA preferences:
- Sign in
- Select Welcome [your name] at the top of the screen
- Under your name, select Two-Factor authentication.
- Select Enable to turn on, or Disable to turn off, then click Save.
