Grocery Benefit (SSBCI)

Are you a Community Health Plan of Washington (CHPW) Medicare Advantage Dual Complete (HMO D-SNP) member with a chronic condition?

You may qualify for a grocery benefit if eligible for Special Supplemental Benefits for the Chronically Ill (SSBCI). This benefit provides $85 a month to spend on healthy food at participating stores.**

Who is eligible for the grocery benefit?

To be eligible for SSBCI and the grocery benefit, you must be enrolled in a CHPW Medicare Advantage Dual Complete plan and be diagnosed with one or more of these chronic conditions:

  • Autoimmune disease
  • Cancer
  • Cardiovascular disorders
  • Chronic alcohol and substance use disorders
  • Chronic lung disease
  • Chronic and disabling mental health disorders
  • Dementia
  • Diabetes mellitus
  • Heart failure
  • HIV/AIDS
  • End stage liver disease
  • End stage renal disease
  • Neurologic disorders
  • Severe hematologic disorders
  • Stroke

You must also have experienced one of the following:

  • Multiple hospital stays or emergency room visits over the past year
  • Regular use of emergency services combined with behavioral health needs
  • Many visits to multiple specialists, showing you have complex care needs

What is the grocery benefit?

Eligible members will receive $85 dollars per month to spend on healthy food at participating stores. If you qualify for SSBCI, you will get a grocery card that these funds are added to each month.

What can I buy with my grocery card?

You can buy healthy foods, prepared meals, fresh produce, and other eligible items from participating stores.

Items include:

  • Bread
  • Fish
  • Meat
  • Herbs, spices, and extracts
  • Meat substitutes
  • Milk
  • Vegetables
  • Pasta and noodles
  • Processed cereal products
  • Yogurt
  • And more

How do I sign up?

You will need to work with CHPW’s Care Management team to confirm your eligibility. If eligible, you will be automatically signed up to receive the SSBCI grocery benefit.

To get started, contact our Care Management team at 1-866-418-7005 Monday through Friday from 8 a.m. to 5 p.m.

How do I activate my grocery card?

After you receive your SSBCI grocery card in the mail, activate it online at CHPW.NationsBenefits.com/Activate or call 1-877-205-8005 (TTY: 711).

Once received, your card should look like this (front and back):

Activate card

Check your balance and transaction history using your Benefits Pro Portal at CHPW.NationsBenefits.com or the Benefits Pro App.

To register, you will need your CHPW Member ID on your CHPW Member ID Card.

How do I use my grocery benefit? Where can I use my grocery card?

If eligible, your prepaid grocery card will come in the mail from CHPW’s partner, NationsBenefits.

Each month, $85 will be added to your card to buy eligible grocery items at participating stores. If there is a remaining balance at the end of the month, it will not carry over to the new month.

There are three ways to shop with your grocery card:

  1. Online in your Benefits Pro Portal or using the Benefits Pro® App
  2. Online at Walmart.com
  3. In-person at participating stores, including:
  • Walmart®
  • QFC (Quality Food Centers)®
  • Fred Meyer®
  • IGA®
  • Albertsons®
  • Haggen®
  • Safeway®

Can I use my grocery card on Instacart?

To use your grocery card on Instacart, you will need to first login to your Benefits Pro Portal or the Benefits Pro App. There you can convert all or some of your card balance to “Fresh Funds” vouchers each month to shop with on Instacart.

When you are ready to shop, enter your voucher code at instacart.com/freshfunds/nations_benefits.
