什么是护理管理？
Managing your health all by yourself can be challenging. Care Management means getting extra support in taking care of your health with a dedicated CHPW Care Team that works hand in hand with your Primary Care Provider and clinic to help make sure your needs are met.
Care Management does not cost you anything. It is free to all CHPW members. Think of it as support with a personal touch.
The Care Team
The Care Team includes a Care Advocate, a Care Manager, Clinical Care Coordinators, and Community Health Workers—all working closely together to support you in achieving your health goals.
The Care Management team does not just focus on your medical care, they consider the big picture—your overall health.

| Case Managers and Clinical Care Coordinators | Are licensed clinical professionals who work with you and your provider(s) to coordinate and personalize your care. |
| 社区卫生工作者 | Have lived experiences that allow them to be helpful resources to connect you to important services beyond your health care providers, like food, transportation, and housing. |

How the Care Team works together

Case Manager/Clinical Care Coordinator
The Case Manager and/or the Clinical Care Coordinator work with your doctor and clinic to coordinate appointments and establish a personalized care plan for you

Organizes extra support, like transportation for you to and from your doctors’ appointments and the pharmacy for prescriptions
